Carers and Disability Link provides a range of support and personalised care to people with disability, including social activities, community access, personal care, gardening, shopping support, housekeeping, cooking and transport. Our workers support clients to achieve their goals whilst ensuring the client has control over their choices.
Carers and Disability Link are experienced in providing a range of supports for people with disability and their family members. We are a registered NDIS Provider, which means we can provide support to participants of the NDIS. We also provide supports to people with disability who are not supported by the NDIS. We are a Registered NDIS Provider
The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life. The NDIS also connects anyone with disability to services in their community.
Services Provided
Plan Management
Plan management is a type of disability service funded through the National Disability Insurance Scheme (NDIS). The overarching function of plan management is to assist and support NDIS participants to manage their NDIS plan funding.
Support Coordination
Support coordination is a type of disability service funded through the NDIS. It helps participants to make the best use of their supports in their NDIS plan. Support coordination is a capacity building support which helps participants to:
- Understand and use their NDIS plan to pursue their goals.
- Connect participants with NDIS providers, community, mainstream and other government services.
- Build confidence and skills to use and coordinate their supports.
Shopping Assistance
We provide support to people with disability to plan for their everyday needs, assisting in getting to the store and providing support whilst shopping.
Medication Assistance
We provide support with management of medication. This includes the administration, documentation and distribution of medicines, and handling medication contingencies.
Garden Maintenance
We provide a range of general garden services, including lawn mowing, pruning, weeding, planting and mulching to assist people with disability to maintain their gardens.
Social Activities
We provide supports to assist people with disability to access their communities and participate in social, economic and daily life activities.
Daily Living Skills
We can provide assistance or supervision of personal tasks during day-to-day life that enable people with disability to live as independently as possible. These supports are provided individually and can be provided in a range of environments, including in-home.
Personal Care
We can assist people with disability with personal care, including hygiene (e.g. showering, toileting including managing continence needs) and personal appearance (e.g. dressing, shaving or other grooming).
House Cleaning
We provide supports to assist people with disability to clean and maintain their home.
Transport
We can arrange transport, supporting people with disability to stay connected to their communities.
Short Term Accommodation
We have a 4-bedroom, 4-bathroom house located in Clare for short term stay or respite. Meals, activities and 24-hour support is available.
